Expose & bond for impacted teeth

Impacted teeth — especially maxillary canines — may require surgical exposure to allow your orthodontist to guide the tooth into position. We expose the tooth, place an attachment when appropriate, and coordinate the plan with your orthodontic team.

Surgical luxation & orthodontic assistance

Some teeth are slow to move due to dense bone, position, or partial ankylosis. In select cases, surgical assistance (including gentle luxation or exposure) may help facilitate orthodontic movement while protecting adjacent roots and periodontal support.

Orthodontic‑directed extractions (premolars and select teeth)

When recommended by your orthodontist, removing specific teeth can create space to relieve crowding, improve bite alignment, or guide eruption. We coordinate timing and tooth selection closely with your orthodontic plan to keep treatment efficient and predictable.

  • Upper and/or lower premolar extractions when space is needed
  • Removal of retained baby teeth or teeth interfering with eruption
  • Comfort‑forward care with local anesthesia and sedation options when appropriate

Tooth uprighting & assistance for difficult movement

Teeth can tip into spaces, remain partially buried, or resist movement. In select cases, minor surgical assistance may support your orthodontist’s mechanics — for example to help upright a tipped molar, improve access for traction, or remove obstacles that block movement. Candidacy depends on root anatomy, bone support, and periodontal considerations.

Coordinated care with your orthodontist

  • Pre-surgical planning based on imaging and orthodontic goals
  • Precise access that preserves gum tissue and supports esthetics
  • Comfort-forward care with office‑based IV sedation and local anesthesia options
Goal: help your orthodontic plan succeed with the least invasive surgical support necessary.

Frequently asked questions

What is an “expose & bond” procedure?

It is a minor surgery to uncover an impacted tooth and place an orthodontic attachment so your orthodontist can guide the tooth into position.

Is expose & bond done under sedation?

Many patients prefer IV sedation for comfort and anxiety control. In some cases, local anesthesia (with or without nitrous) may also be appropriate depending on the procedure and your preferences.

How long does recovery take?

Most patients have mild swelling and soreness for a few days. We’ll provide aftercare instructions and coordinate timing with your orthodontist.

Will you coordinate directly with my orthodontist?

Yes. We routinely coordinate timing, attachment preferences, and surgical approach with orthodontic offices to keep treatment efficient.

What if the tooth is ankylosed?

If a tooth is fused to bone (ankylosed), movement can be limited. We’ll review imaging and discuss options with your orthodontist, which may include surgical assistance or alternative plans.

Can you help with teeth that won’t move or need “loosening”?

In select cases, gentle surgical luxation or exposure may help. Candidacy depends on tooth position, root anatomy, and periodontal considerations.

Will there be stitches?

Often yes. Sutures may be removed at a follow-up visit or may dissolve, depending on the approach and materials used.

Do you remove premolars for braces or aligners?

In some orthodontic plans, premolar extractions are recommended to create space and improve alignment. We confirm the exact teeth and timing with your orthodontist before surgery.

Can you help upright a tipped molar or a tooth that’s hard to move?

Sometimes. In select cases, surgical assistance may support orthodontic movement. We’ll review imaging and coordinate with your orthodontist to determine the safest, most predictable approach.
